Whether you’re visiting your primary care doctor or having an unexpected ER visit, we are with you each step of the way. We will provide education and information or assist in communication between multiple health facilities.

- 24/7 Emergency Support- We will send your personalized reports of your health history to the hospital or provide your Moritrac Nurse to communicate a live report to your medical providers by phone

- We will help communicate health changes with you and your family together
- Rapid review and communication from your Moritrac Liasion Team when critical data is identified

What will happen once I become a member?
- Your Moritrac Liasion will provide a peaceful onboarding, and you will have immediate access to your team for emergencies or question
- You will provide data on your current healthcare providers and recent health events
- We will request records from all previous healthcare providers
- Your Moritrac Team will create custom tracking and reports on your health history
- Your first Moritrac Health Conference will be scheduled
- You attend your Moritrac Health Conference and receive your first round of custom reports and education.
